There is a new process for sports physicals.
- Bring the "Preparticipation Physical Evaluation - Physical Exam Form" and the "Preparticipation Physical Form- History Form" to your child's physician and have it completed. This completed form stays with your child's doctor.
- Ask your child's doctor to complete the "Medical Eligibility Form" and return it to the health office.
- If your child's completed physical form is between 90-365 days old, you can complete the "Health History Questionnaire" and submit it to the health office.
Attached are any necessary forms. The deadline to submit sports forms is October 28, 2024. Our school physician needs time to review these forms and officially clear your child for tryouts on November 13th. Any forms submitted after the deadline may not be approved in time for tryouts.
